JOB SUMMARY:

The Companion provides personal care to assigned residents under the direction of the Licensed Practical Nurse or Client Care Coordinator in accordance with the Care Plan. The Companion provides support, advocacy, and choice to Residents and their families to ensure the highest quality of resident focused care achievable within the Mission, Vision and Values of Care At Home. Demonstrates ongoing competence utilizing knowledge, skill, judgment and evidence based practice in caring for the older adult. Ensures compliance with continuing care health service standards, employment and safety-related statutes, legislation, codes and regulations and CAH policies.  Provides support to the residents and families in an environment that promotes a state of wellness, independence and person-centered care.

KEY RESPONSIBILITIES:

Participates in the development, implementation, evaluation and revision of the resident care plan as appropriate.  Carries out all required activities as assigned to provide holistic care to residents such as dietary and recreational duties.
May include dietary duties as assigned to ensure the nutritional needs of residents are being met.
Observes the residents’ physical condition, behavior and mood and promptly reports any changes in the residents’ status to the office.
Engaging clients by conversing with them and planning and attending them as they participate in appropriate social activities and hobbies.
Ensures effective and timely documentation
Handling basic housekeeping, such as dusting, sweeping, mopping, organizing, and laundry.
Providing emotional support for clients as they heal or cope with their conditions.
Handling errands for or with client, such as grocery shopping, paying bills, writing letters, etc.
Driving clients to planned outings or events.
Participates in the development of plans, objectives and strategies for care services, which are aligned with overall organizational strategic directions.
Participates in the development, revision and implementation of operational policies, procedures and programs in collaboration with appropriate team member..
Promotes a safe work environment for residents, family members, co-workers and self.  Reports unsafe conditions within the living and working environment. Takes required action to decrease the risk of injury immediately and/or notify the Site Supervisor of the concern.
Represents and participates in corporate or site committees as appropriate.
Participates in performance measurement to achieve continuous quality improvement. 
Actively supports, promotes and implements the mission, vision, values and strategic directions of Care At Home.
 

JOB SPECIFICATIONS/QUALIFICATIONS

 Minimum Recruitment Standards:

High School Diploma or equivalent.
More education, experience, specific skills or interests, or training may be preferred.
Excellent references and a valid driver’s license with a clean driving record.
Strong communication, listening, planning, housekeeping, and reasoning skills.
Ability to handle physical demands, such as lifting or support client and stand, walk, bend, or crawl for extended periods.
Current CPR and First aid

Must be willing to work anywhere in Edmonton
Must have a driver’s license and a reliable vehicle

Caring, sympathetic approach to working with clients.
Willingness to follow directions and adhere to all company policies, safety procedures, and dietary, exercise, and medication plans.
Experience working with the elderly, chronically ill or the disabled.
Demonstrated skills in communication, teamwork, and customer service..
Awareness of accident and injury prevention, hazard identification and control; adherence to safe work practices and procedures is essential in this position.
Demonstrated knowledge and commitment to person-centered care, and Residents’ Rights.
Demonstrated commitment to the mission, vision and values of the Care At Home is essential in this position.
